CD4 count before ARV 39
Taking ARV since 2 years. recent CD4 264
Observation: Emaciated, wearing scarf.
Herpes zoster 1994 lower back and chest
repeat 1998 and again 2000 and 2002, four times
2004 TB
2006 had blood check which proved HIV positive.
Had allopathic TB treatment and developed rashes(- normal reactions to TB drugs).
Does not have malaria now, more before ARVs.
But it was bad malaria, even with scanty count of parasites she was in a bad condition.
When there is conflict she wakes at 1-4 am.
She struggles hard to build a house, but one of her children threaten to poison her to get the property. Her son is very rude to her.
Crying when she is talking of this. Says it is deeply emotional.
Husband is promiscuous, works in mines, many partners, she got the AIDS from him.
Pain in heart when she thinks about these things. She cries a lot. Likes crying and consolation.
She talks to her sister and feels better after.
Observation: she laughs when talking about painful things.
Fear of thunderstorm (Unusual in TZ).
One still born baby from toxemia.
Dream of walking along the river bank. Its with difficulty as if she will just fall in the water but keeps walking along the bank. She tries her best to avoid tipping over and falling in.
Dream of climbing rocky mountains with difficulty but manages to go all the way up, sometimes she climbs down too, very high, difficult, but she does it.
(JS analysis- pushing against difficulties, against a border, against son)
Dreams of friends and parents who have died and they celebrate together.
Tongue geographical, cracked.
Epigastric hernia 1982.
Aversion coffee and tea.
Frequent urination night.
Prefers hot to cold weather (Unusual TZ).
Joint pains, mild, in fingers and wrists and feet > hot applications > massage.
Very giving and generous and sympathetic (JS - this is common to every patient I have seen so far, a Tanzanian thing)
Appetite low.
Chose rubrics:

Analysis:
I chose Rhus-Tox because of the dreams of drowning (suicidal by drowning) and the dreams of climbing with effort. Also the frequent herpes zoster, skin rash, painful joints and the delusion or reality of her son wanting to poison her. It was not a remedy I was expecting for AIDS, may be more constitutional, time will tell with CD4 effect. There is a water theme with the river and frequent urination.
Rx: Rhus -tox 12c daily
Follow-up 12.08 two weeks later
The skin is smoother and better
sleep is much better
dreams are gone >>
eating is much better>>
the thoughts at night are better
she is more cooperative, the son is nicer and asked if he could bring her cement for the house
urination night is >>
happier, very happy! Feel untroubled and symptom free.
rx: continue rhus tox 12c daily
